This page helps you and your prescriber notice patterns across the month.


It tracks medication benefits, medication costs, and context in a simple weekly view.

Mark how helpful medication felt overall for:

  • Overall effectiveness

  • Focus/ attention

  • Hyperactivity

  • Task Completion

  • Impulsivity 

 

More filled = more helpful that week.

Medication Costs


Mark if each cost was present during the week.
 

Leave blank if not present.

 

Costs include:

  • Heart palpitations 

  • Noticeable symptom rebound (when meds wear off)

  • Trouble sleeping

  • Was more irritable 

  • Loss of appetite

  • whether barriers were about focus, anxiety, perfectionism, or energy

  • one small adjustment for next week

 This tool does not replace medical advice. 

 It helps make patterns easier to see and discuss. 

 Not every difficult week means medication is wrong. 

 Sleep, stress, health, and emotional load can all affect how  medication feels. 

Bring it to your appointments

Bring your completed map to:

  • Psychiatry reviews

  • GP visits

  • Psychology sessions

 

Patterns across time are more useful than single bad days.
